What should I bring for my first appointment?
Please bring with you any recent labs, insurance cards, and other pertinent information you feel would help us in better treating your health concerns. Please try and submit your completed patient packet one week prior to your scheduled appointment time so that this can be fully reviewed, as this is a vital source of information! Also it is very helpful to write down any questions you would like to discuss during your appointment so that all of your concerns can be addressed.
What is your cancellation policy?
Please provide a minimum of 24 hours notice when cancelling appointments. We understand at times there can be extenuating circumstances and will work with you as best we can on rescheduling your appointment time. Thank you in advance!
Why are appointments not filed on insurance?
Unfortunately insurance does not cover functional medicine at this time and some providers attempting to file insurance are being asked to return payments due to not following what is considered to be standard of care. However, the amount of time defined on our appointment scheduling is your time and this will not be cut short. You will still be able to use your insurance for labs and medications ordered for you. Also we would be happy to provide you with an explanation of benefits to present to your insurance company for potential reimbursement of the cost of appointments.
